Consent and Release Form

To be completed by a parent or guardian if student is under the age of 18.

I, the undersigned parent(s) or guardian(s), consent to my child, listed above as STUDENT and who is under the age of 18, or I, the STUDENT as an adult of the age of 18 or older, consent to participating in the activities connected with Campus Days, an event sponsored by SOUTHWESTERN ASSEMBLIES OF GOD UNIVERSITY.

If the STUDENT has medical conditions, which may be relevant to a physician if an emergency occurs, I have listed them below. If I cannot be reached within a reasonable period of time, as determined by university officials, I authorize the university or the adult sponsors to make emergency medical decisions for my child. If there are any activities I do not want my child to be involved in, I have listed them below.

I UNDERSTAND AND AGREE TO ASSUME ALL OF THE RISKS WHICH MAY BE ENCOUNTERED ON SAID ACTIVITIES, INCLUDING ACTIVITIES PRELIMINARY AND AFTER THE EVENTS. These activities may include but are not limited to inflatable/mechanical obstacle course, inflatable/mechanical bounce house, inflatable/mechanical bull, inflatable/mechanical meltdown, inflatable/mechanical hamster balls, rock climbing wall, zip line, archery tag, Euro bungee, yard games, and sand volleyball. I do, for myself and for my child, heirs and assigns, irrevocably and unconditionally release, acquit and forever discharge SOUTHWESTERN ASSEMBLIES OF GOD UNIVERSITY and its agents, employees, and volunteers from any liability, actions, causes of actions, claims, expenses, obligations, and damages of any nature, which I now have or which may arise in the future, in connection with my child’s participation in the described activity or in any other associated activities including, but not limited to, any injury to my child or property, even injury resulting in death.

I expressly agree that this release, waiver, and indemnity agreement is intended to be broad and inclusive as permitted by the law of the State of Texas and that if any portion is held invalid, the balance shall, notwithstanding, continue in full legal force and effect.

I expressly agree that any dispute arising from this activity shall be submitted to binding arbitration, to the extent it is not covered by insurance. This release contains the entire agreement between the parties. I further state that I HAVE CAREFULLY READ AND UNDERSTAND THIS RELEASE AND KNOW THE CONTENTS HEREOF, AND I SIGN THIS RELEASE AS MY FREE ACT.

This Consent and Release constitutes a legally binding agreement.
